扩张型心肌病的症状

扩张型心肌病早期症状隐匿，常见活动后气短、乏力、下肢水肿，随病情进展可出现夜间憋醒、端坐呼吸、心律失常等表现，严重时可突发心力衰竭。

一、早期症状特点

早期症状不典型，多表现为活动耐力下降（如爬楼梯、快走后易疲劳）、轻微气短（尤其上楼或运动后明显）、乏力（日常活动后需长时间休息恢复）。部分患者可能仅有不明原因的体重轻微增加（提示早期水钠潴留），易被误认为“亚健康”状态而延误诊治。

二、典型进展期症状

随着心脏扩大和心功能下降，症状逐渐加重：

• 呼吸困难：平卧时加重，需垫高枕头或坐起缓解（夜间阵发性呼吸困难），严重时静息状态下也感气短；

• 水肿：从脚踝、小腿开始，逐渐蔓延至全身，按压皮肤会出现凹陷且不易恢复；

• 乏力与头晕：心脏泵血不足导致全身供血减少，常伴随活动后心悸、头晕甚至晕厥；

• 心律失常表现：可出现早搏、房颤等，表现为心慌、心跳不齐或脉搏忽快忽慢。

三、并发症相关症状

疾病晚期易合并严重并发症：

• 心力衰竭急性发作：突发严重呼吸困难、端坐呼吸、咳粉红色泡沫痰，需紧急就医；

• 血栓栓塞：心房扩大易形成血栓，脱落可导致脑栓塞（突发肢体瘫痪、言语不清）、肺栓塞（胸痛、咯血）等；

• 心律失常猝死：部分患者因恶性心律失常（如室速、室颤）可发生猝死，需警惕不明原因晕厥。

四、特殊人群表现差异

• 儿童患者：婴幼儿可能表现为喂养困难、生长发育迟缓、反复呼吸道感染；学龄儿童出现活动耐力差、运动后气促、生长发育落后于同龄儿童；

• 老年患者：症状易与高血压、冠心病等老年常见病混淆，需结合心脏超声等检查鉴别；

• 女性患者：早期症状可能更隐匿，绝经后女性因激素变化，心功能下降速度可能加快。

扩张型心肌病症状具有渐进性和非特异性特点，若出现上述症状且持续加重，应尽早到心内科就诊，通过心脏超声、心电图等检查明确诊断。严禁自行服用任何药物，需在医生指导下规范治疗以延缓疾病进展。
